If the death happens at home

  1. Phone the surgery (or 111 when we are closed). A doctor will confirm the death.
  2. Contact a funeral director.
  3. Medical certificate: the doctor completes the medical certificate of cause of death. We will tell you when it's ready and how it reaches the registrar.
  4. Register the death with the register office, usually within 5 days. The registrar gives you the documents your funeral director needs. Register a death (GOV.UK)

If the death happens in hospital

Contact a funeral director. The hospital bereavement office will guide you through the certificate and registration.

Cremation

Your funeral director usually arranges any extra paperwork with us directly.

Tell Us Once

The Tell Us Once service lets you report a death to most government organisations in one go.
